Opening a Trust Account
If you’re interested in opening a Trust account, please complete the following steps.
Trust or other applicants using an Employer Identification Number (EIN) or Tax ID are not eligible to apply online. One of our retail branches will be happy to help you explore other available account options.
Please Note: eOne Savings and Checking accounts are currently available online only to consumers applying with a Social Security Number (SSN).
What Salem Five will need from you:

- Your completed Trust Application
- Completed W‑9 Form
- Completed Delegation of Trustees Form
- A copy of a government-issued ID (driver’s license or state ID)
- A notarized copy of the Certificate of Trust or Trust document
- Your funding check (minimum $10 per account)

Please note:
- Additional identifying documentation may be required
- Allow up to 10 business days after receipt of application
- Please review our Consumer Banking Services Agreement and Account Disclosures
- By submitting the application, you authorize Salem Five to obtain credit and reference information from consumer reporting agencies for evaluation.
